Do you believe that food does not have an impact on MS? They tell you that there are no studies showing a connection between MS and diet? They're wrong!

Since more than a decade, I have conducted clinical research to test the effects of diet on people with MS. It's good to know that people are finally beginning to use diets as a way of controlling MS symptoms. Our research lab has studied the modified Paleolithic Diet, the ketogenic Diet, and the Low Saturated Fat diet.1-7 It is clear that the improved diet results in a reduction of fatigue, anxiety, depression4, and depression4 as well as an improvement of quality-of-life.1,2,4,6,7I have conducted seven clinical trials and published more than 70 peer reviewed scientific papers, abstracts and posters. Other researchers have cited our MS diet studies hundreds of times.

There are more people who are conducting dietary intervention studies for MS. Twelve randomized controlled dietary interventions studies have been published in people with MS. A recent editorial in Neurology, one of the most influential and widely read journals that publish MS research, stated that there are now proofs that diet can improve quality-of-life for people with MS. This is huge. Your physician may not be up to date with the latest studies if they tell you that diet doesn't matter.

This article will explain how you can sign up to participate in the study.

Diet studies in MS findings

Multiple studies combined and analyzed in a single analysis provide the strongest evidence of whether an intervention is beneficial (or harmful). Meta-analysis is the name of this type of analysis. A network meta analysis compares and combines all studies known to determine the most effective treatments. This is a much stronger proof than just looking at one study. Now that we have published enough studies on the impact of diet on fatigue, and on quality of life, it is possible to perform these important meta analyses.

This paper was a network metaanalysis. The investigator can rank interventions in order of effectiveness using a network meta-analysis. In January 2023, the paper was published by Neurology - the journal most read by neuroscientists, practicing neurologists and neurosurgeons. Dr. Linda Snetselaar reviewed randomized dietary interventions in MS with fatigue or quality-of-life as the outcome. The authors identified 12 studies that examined 8 diets, including Mediterranean, Paleolithic (low saturated fat), anti-inflammatory diets, ketogenic diets, fasting diets, calorie restrictions, and the participant's normal diet. These 12 studies enrolled 608 participants in total. For the improvement in fatigue, mental quality of health, and physical quality of health, we calculated the standardized mean differences (SMD). For the control diet and intervention diet, the 95% confidence range (CI) was calculated. Scientists can say that the intervention is successful if both the SMD improvement and the 95% Confidence Interval are on the intervention's side. Dr. Snetselaar used this method to determine if any diets are effective in reducing fatigue or improving the quality of life. This is how she ranked all eight diets in terms of their effectiveness at reducing fatigue and improving quality of living.

Comparing each diet to the control, the confidence intervals for three diets were completely on the side of fatigue reduction. The five other diets did not perform significantly better than control diet. In descending order of magnitude, the Paleolithic diet (SMD -1.27, 95% CI -1.81, -7.74), low saturated fat (Swank Diet) (SMD -9.0, 95% CI -1.39., -04.22), and Mediterranean diets (SMD -8.89, 95% CI -1.15., -6.64), were all associated with a reduction in fatigue severity. Two diets were significantly associated with improved quality of living when compared to the usual diet. These two diets are the Paleolithic diet (also called the Wahls(tm), diet) (SMD 1,01; 95%CI 0,40, 1.63), and the Mediterranean diet (SMD 0,47; 95%CI, 0.08, 0.86). Our lab was responsible for half of the Swank diet studies and all the Paleolithic studies that were cited in the meta-analysis. The Paleolithic Diet that was reported is actually the Modified Paleolithic Diet, also called the Wahls(tm).

How to eat if you suffer from MS

Three of the best diets to reduce fatigue or improve quality of life include the Mediterranean diet, the modified Paleolithic diet (WahlsTM diet), and a diet low in saturated fats (Swank Diet).

The Mediterranean diet encourages the consumption of more fruits and vegetables, whole grains, legumes and fish. It also reduces or eliminates processed foods, fast food, white rice and breads. The Mediterranean diet also limits dairy and red meat consumption.

The Swank diet is a low-saturated fat diet that encourages the consumption of vegetables, fruits and whole grains as well as white poultry and fish. It restricts saturated fat intake to 15 grams per day and restricts red and dark meats, dairy fat and saturated fat.

The modified Paleolithic Diet (Wahls(tm), encourages eating more vegetables (6 to 9 servings a day), 6-12 ounces of fish or meat per day and fermented food, nuts and seeds every day. The diet excludes grains containing gluten, dairy and eggs, and reduces or removes processed foods and fast food.

Next steps for improving your diet

It is hard to stop eating processed foods and fast food. They are all around us. They are inexpensive. If you try to stop eating fast food and processed foods, you may experience headaches, irritability and intense cravings. This is because these foods are designed to cause food cravings.

I tell my clients that if the family makes dietary changes together, they are more likely to succeed in changing their eating habits. Discuss which foods you would like to remove from your diet. Start with sugar-sweetened drinks and processed snacks. You can start planning for success once you and your family decide which foods you will emphasize, and which you'll reduce or eliminate.

Create an eating environment that supports your goals. You can make it easier for people to eat foods that you want them to. And you can remove food items from sight so they are not tempted.

It's not easy to give up sugar, gluten, milk, and processed food. Gluten (protein found in wheat, barley and other ancient grains), and casein, (protein from dairy), stimulate endorphin receptors, which stimulate pleasure and attention centres in the brain. People may feel irritability and cravings as they reduce or eliminate processed foods and sugar. The first thing you should do is eliminate all sugar-sweetened drinks. The next step is to eliminate foods that cause blood sugar levels to spike the fastest, such as white bread, pastas, cereals, and whipped potatoes. Third, reduce fast food intake or eliminate it altogether. These initial steps are part of the recommendations made for the three most efficient diets: Mediterranean, modified Paleolithic and low saturated fat.

The modified Paleolithic (WahlsTM) diet and the Mediterranean diets share many features. These include less sugar added, fewer ultra-processed food, and a greater amount of non-starchy veggies. Low saturated fat diets also reduce added sugar, ultraprocessed food, and fast food. Start by eliminating added sugars, ultra-processed snacks, and fast food.

It is not surprising that Dr. Snetselaar found the greatest effect with the modified Paleolithic (WahlsTM) diet. We have studied the modified Paleolithic Diet in several studies. It removes three of the most common antigens that trigger the immune system (gluten casein and egg albumin).

In the same issue as Dr. Snetselaar’s network meta-analysis, an editorial entitled Diet and Multiple Sclerosis appeared. The authors of the article recommended that neurologists discuss the importance of diet with all of their MS patients. The authors of the editorial said that MS patients need to be informed that a healthy eating plan is important and can affect fatigue and quality-of-life. The MS group also recommended that MS patients visit a Registered Dietitian for personalized advice and support on diet improvement. These visits would be likely covered by most health insurance plans. They also suggested that MS patients be sent to a Registered Dietitian for personalized guidance and support regarding diet improvement. These visits would likely be covered by many health insurance plans.

We've come a very long way. In 2010 my ideas about diet were radical. My ideas were deemed dangerous and I was prohibited from speaking. The modified Paleolithic Diet was once considered dangerous for MS Patients. Well-respected MS scientists now say that all MS patients need to be encouraged to adopt a healthy, low-saturated fat diet, such as the Mediterranean diet, Paleolithic diet, or a Paleolithic diet.

Take part in a study

It is a great step forward, but we still need larger and longer diet studies. We are looking for people who want to participate in these studies.

We are recruiting participants to participate in a study that compares the modified Paleolithic Elimination Diet and the time-restricted Ketogenic Diet with the usual diet over a period of two years (NCT05007483). We are seeking people between 18 and 70 years old with relapsing remitting MS. The participants must agree to randomly be assigned to the modified Paleolithic diet (also known as the Wahls diet), the time-restricted Olive Oil Ketogenic diet or the usual diet. If you are currently following a Paleolithic or ketogenic diet, then you will need to be willing to change to the modified Paleolithic or ketogenic diet if randomly assigned to it.

You will also need to be willing to visit Iowa in the month of 0 to conduct your first study. You will need to return to Iowa in month 3, and also month 24. You will be asked to complete questionnaires regarding your mood, energy and ability to perform daily tasks. Also, you will be tested on walking, hand, memory and vision. You will be given a brain MRI without contrast at month 0 as well as month 24 to measure the rate of change in brain volume.

We are interested in whether improving the diet can lead to a healthier brain ageing. The average rate of brain volume reduction in people with MS is three times faster than that seen with healthy aging. My clinical experience with MS patients has shown me that improving diet can help people regain healthy brain volume loss. It will be a huge achievement if our study proves that. None of the DMTs has been able get people with MS to return to healthy brain volume losses.

We want you to be part of this study if you have relapsing remitting MS and you are between 18 and 70 years old. You must also be willing to visit Iowa three times in two years. Please complete the screening tool if you're interested. We encourage you to join our patient registry even if you don't qualify for the study. I urge you to fill out this short survey if you suffer from optic neuritis (also known as radiologically or clinically isolated syndrome), multiple sclerosis, relapsing remitting MS, or both.

It takes 30 years to change the standard of health care. Halfway through my journey, I have been on this path for 15 years. My goal is to make diet the standard of care for MS patients, so they are told diet is important and encouraged to improve their diet in order to protect their brains. We need to publish more peer-reviewed, scientific studies in order for this to be possible.
